After two days of plugging and unplugging the mouse, taking the
batteries in and out, moving the receiver, I finally broke down and sent
the above message.

5 minutes later, I moved the receiver one more time, replaced the
batteries one more time, et voila!

I have no idea what I did to make it work.  Probably just moving the
receiver sufficiently, but most likely the cause of everything working
was the fact that I sent the message to -questions...
