Make sure your kernel is in sync with ipfw. Assuming you have recently
rebuilt your kernel, you may have to re-install the .h files from
/usr/src/include and rebuild the ipfw program (/usr/src/sbin/ipfw).

Sorry if you've done this already, I've missed half of this thread, but
I had a similar problem and rebuilding ipfw fixed it.
