You have a hardware problem; either a faulty busmastering disk controller
(less likely) or bad memory (most likely).

If at all possible, wind your motherboard waitstate settings right out
to the maximum.  If the problem persists, you'll have to replace various
bits until you find the culprit.

You may also want to try disabling caching (both in the processor and on
the board) if you can, as this problem can also be caused by faulty cache
hardware.

You used fips to shrink your DOS partition prior to installing, and you
caused it to move from one clustersize range to another. 

_DANGER_.  This is believed to be a trigger to a _nasty_ bug in the msdosfs
code that can silently corrupt your BSD partition.  Avoid using the 
msdosfs whenever possible.
