You go to the relevant mod_phpX directory and type make install.
I.e. /usr/ports/www/mod_php3 (or 4). That's all there is to it.
It gives you a menu of choices and you check the relevant ones
like mysql.

It gets more interesting when you want to add more modules
dealing with MySQL. Haven't delved into that, yet <g>.
