Don't worry if patch doesn't remove the directory - committers have to
explicitly "cvs remove" deleted files and directories.  As the
"Upgrading" sections of the Porter's handbook notes, be sure to
mention any files added or deleted when sending the PR.

patch(1) works this way - it doesn't matter.

There is - section 14, "Upgrading".  However, it could contain more
information or possibly be written more clearly.  If you have any
suggestions on how to improve it, please let us know.
