I'm using qmail-scanner (a qmail add-on available
at http://qmail-scanner.sourceforge.net/) for email
filtering, without an external virus DB, and
i'm happy with it.

We only use it as simple manner of reacting to well known
viruses, through regular expression hand configuration.

You could use a full-blown virus DB, but in this case you
might need more processor, keep close control on
DB synchronization, and to remember that emails could
be ciphered end-to-end, defeating the most clever Mail
Transfer Agent anti-virus software.
