I assume that the source is at
  https://svnweb.freebsd.org/base/head/usr.sbin/makefs/

There i can see the reason for problem #1 (unitialized malloc
memory in date field) and that my assessment of problem #3
was wrong (two bugs, one partly correcting the other).
