Turn out this was operator error: I neglected to "connect" the mouse to the
base. One needs to press the "connect" buttons on both the mouse and the base
to do this; after I did that the mouse worked fine.

My /etc/X11/XF86Config has

Section "InputDevice"
    Identifier  "Mouse0"
    Driver      "mouse"
    Option      "Protocol" "Auto"
    Option      "Device" "/dev/sysmouse"
    # Mouse wheel support
    Option      "ZAxisMapping" "4 5"

and scrolling seemed to work fine in XFree86-4/KDE.
