Anyone know what could have caused this?

This happened during the news expire run, and the filesystem question
does not have softupdates enabled (at least, I'm assuming the filesystem
this happened on is the one I think it is, I'd probably have to dig
about more than I'd like to to find out for sure). If anyone wants more
information, let me know and I'll try to find it. (Of course, it may
well be dodgy hardware, but I thought I'd report it anyway in case it's
a problem someone recognises.)
