Most of the 4-port NICs I've seen use the DEC Tulip 21x4x, which has polling 
support but not VLAN_MTU or VLAN_TAGGING.  I've seen one-quad port fxp using 
Intel chipset which had polling and VLAN_MTU, but not VLAN_TAGGING.

A quick look on CDW comes up with:

Intel PRO/1000 MT Quad Port Server Adapter ($500)

...which ought to do, or even:

Sun Quad FastEthernet network adapter - 4 ports ($1600)

...which is rather expensive.  :-)

There's also a Adaptec ANA-64044 LV KIT 4 port 66MHZ NIC ($480)

...but I don't know what chipset that is using.
