Are you starting qmail-smtpd from inetd or tcpserver?

If tcpserver, you can use the -H flag to disable looking up the host
name, and -R to disable the ident lookup.

Alternately, make sure that you aren't blocking the ident lookup with a
firewall.  It is 113/udp.
