I second nagios as well , solid and very easy to configure.
if you feel like you need nice graphs then zabbix ( in  the ports as 
well ) is a nice mysql/php tool you can use .
there is a bit of a learnign curve and the rc scripts provided with the 
ports need some hacking but its a nice tool .
